Once the registration documents have been fully completed and submitted, please gather the following documents and bring them to your child’s school:
1. Proof of Residency - acceptable documentation includes the following:
- A Current Deed
- A Mortgage Statement (no more than 3 months old)
- An Active Residential Lease Agreement
- A Utility Bill (no more than 3 months old)
- A Credit Card Bill (no more than 3 months old)
- A Bank Statement (no more than 3 months old)
- A State Property Tax Bill (no more than 3 months old)
- An Insurance Document (no more than 3 months old)
- A Piece of Mail Sent in the Past 3 Months by the Federal or State Government such as Benefits Information, Voter Registration, or Tax Documents
2. Photo Identification of Parent/Legal Guardian:
- Driver's License
- RealID
- Passport
3. Proof of Age - acceptable documentation includes the following:
- Birth Certificate

- Notarized copy of Birth Certificate
- Baptismal Certificate
- Passport
