Immunization Requirements

ALL IMMUNIZATIONS MUST BE RECEIVED BEFORE THE FIRST DAY OF THE REQUIRED SCHOOL YEAR. If after the first five days of school, immunization records have not been received OR if there is no medical plan to receive the immunizations, students will not be permitted to attend school.

The only exemptions to the school laws for immunizations are: medical reasons; religious beliefs; or philosophical, strong moral or ethical convictions. For more information on the school immunization requirements, please click here.

Immunizations required for all students in Kindergarten through Grade 12
All Grades: Kindergarten through Grade 12

  • 4 doses DPT, or DT Vaccine (Diphtheria/Pertussis/Tetanus) - Last dose on or after the 4th birthday
  • 4 doses Polio Vaccine - Last dose on or after the 4th birthday
  • 2 doses Measles, Mumps, (1 dose Rubella) - Usually given as MMR - 1st dose must be given on or after 1st birthday.
  • 3 doses Hepatitis B vaccine - properly spaced
  • 2 doses Varicella (chickenpox vaccine) or history of disease - 1st dose must be given on or after 1st Birthday.

Students entering Grade 7: In addition to all requirements listed above:

  • 1 dose Tdap (Tetanus, diphtheria, acellular pertussis) * if 5 years has elapsed since last tetanus immunization
  • 1 dose Meningococcal Conjugate Vaccine (MCV)

Students entering Grade 12: In addition to all requirements listed above:

  • 1 dose Meningococcal Conjugate Vaccine (MCV)
