Immunization Requirements
Immunizations required for all students in grade Kindergarten through Grade 12 |

All Grades: Kindergarten through Grade 12 |

| 4 doses | DPT, or DT Vaccine (Diphtheria/Pertussis/Tetanus) Last dose on or after the 4th birthday. |

| 3 doses | Polio Vaccine | |
| 2 doses | Measles, Mumps, (1 dose Rubella) Usually given as MMR 1st dose must be given on or after 1st birthday. |

| 3 doses | Hepatitis B vaccine - properly spaced | |
| 2 doses | Varicella (chickenpox vaccine) or history of disease 1st dose must be given on or after 1st Birthday. |

| Students entering Grade 7: | ||
| In addition to all requirements listed above: | ||
| 1 dose | Tdap (Tetanus, diphtheria, acellular pertussis) * if 5 years has elapsed since last tetanus immunization |

| 1 dose | Meningococcal Conjugate Vaccine (MCV) | |
